In addition there are more rewards when you open a Medium Casket than you would for a simple one (so you could expect items like shield h3, shield H2, or the helm 3). In addition to Rangers Boots in OSRS GP a distinct drop list, you will also locate Spiked Manacles that cost over 1 million. The other rewards aren't as important as the two items mentioned above.

While it does have an identical drop table in regards to value Medium Casket and is a hard difficulty, it also gives you the chance to roll from the extremely rare table. There are a range of expensive items like 3rd Age Equipment and Gilded Parts that have a value of hundreds or even tens million dollars (like 3rd Age Platebody). Additionally, items from a special drop table are worth it because there are four items that are worth more than 1 million gold. In addition you can get more rolls in rewards when you use a Hard Clue than a Medium One and, in general the scrolls will give more drops.

Similar to the previous box, this one also gives you a chance to win rare and unique items that have an estimated value of tens or thousands or even millions. The main difference between this box and the one before it is that you will have higher chance of obtaining Gilded drops, and a less chance of Third Age items.
